Why Health Insurance Claims Get Rejected and How to Avoid It

In short

Health insurance claim rejection reasons explained in simple words, with easy steps to avoid them and what to do if your claim is rejected. Ask for help.

Health insurance claims are usually rejected for a few clear reasons. These include waiting periods, hidden pre-existing conditions, excluded treatment, late intimation, missing documents and a lapsed policy. Most of these can be avoided with simple care.

Here is each reason, and what you can do about it.

Claim made in the waiting period

Every policy has waiting periods. If you claim for an illness during its waiting period, the claim is usually not paid. This can be the initial, specific-illness or pre-existing disease waiting period.

How to avoid it: Know the waiting periods before you buy and plan treatment for after they end, where it is safe to do so. Read more in waiting period in health insurance.

Hidden pre-existing conditions

A pre-existing disease is an illness you had, or knew of, before the policy began. If you do not tell the insurer, it may reject the claim when it finds out. This is called non-disclosure.

After a set number of continuous years, the insurer usually cannot reject a claim for non-disclosure, except for fraud. This is called the moratorium period, and the length is set by rules.

How to avoid it: Fill the form honestly. Add every illness, medicine and surgery.

Excluded treatment

Every policy has an exclusion list. This can include cosmetic treatment and other items shown in the policy. If your treatment is on the list, it will not be paid.

How to avoid it: Read the exclusions before you buy. Ask the hospital and insurer before a planned treatment whether it is covered.

Late intimation

Policies ask you to inform the insurer within a set time of admission, or before it for planned treatment. If you inform too late, the claim can be delayed or rejected.

How to avoid it: Save the helpdesk number. Inform the insurer as soon as you can, even in an emergency. A family member can do this for you.

Missing or wrong documents

A claim needs papers like the claim form, discharge summary, bills, reports and ID. If something is missing, the insurer may ask for it, and the claim can be closed if you do not reply.

How to avoid it: Keep a folder of all papers. Take copies. Answer insurer questions quickly. Our guide on cashless vs reimbursement claims lists the usual documents.

Policy lapse

If you miss the renewal and the grace period ends, the policy lapses. A claim made after a lapse is not paid. You may also lose benefits you built up.

How to avoid it: Renew early. Set reminders. Pay by auto-debit if you are comfortable with it.

Claims partly paid, not rejected

Sometimes a claim is paid in part. The reasons can be:

  • Room rent limit: Other charges are cut when you take a costlier room.
  • Co-payment: You pay a share of the claim.
  • Sub-limits: A cap on certain treatments.
  • Deductible: You pay the first part.
  • Non-covered items: Some hospital items are not paid.

How to avoid it: Know these terms in your plan. See how to choose a health insurance plan.

What to do if your claim is rejected

  1. Read the rejection letter. It should state the reason.
  2. Check your policy wording against that reason.
  3. If you believe the reason is wrong, write to the insurer with proof and ask for a review.
  4. If you are not satisfied, use the insurer's grievance process. Then you can take the matter to the insurance ombudsman or other official routes. Check current rules.

Do not give up too soon. But also be honest, because a false claim can cause bigger trouble.

A simple habit that helps

Keep one folder, on paper or on your phone, with the policy, the health card, past reports and old bills. Share the folder location with one trusted family member. When a claim comes, you will not need to search for papers in a hurry.

Also read your policy once every year, when you renew. Plans and terms can change, and a quick read helps you remember the waiting periods, limits and exclusions that apply to you. If a term is unclear, ask the insurer or your advisor before you need to claim, not after.

FAQs

Can a health claim be rejected after cashless approval?

Pre-authorisation is not a promise of full payment. The final bill is checked against the policy, and some items may not be paid.

Will the insurer reject my claim if I forgot to tell about a small illness?

It can, if the illness is linked to the claim. Always disclose everything in the form.

Can I get my premium back if the claim is rejected?

Usually no. Premium is the cost of keeping the cover. Check your policy wording.

Does a rejected claim cancel my policy?

Not by itself. But if the insurer finds fraud or serious non-disclosure, it may take stronger action as per the policy.
